I live in Kansas, but I use ViaTalk which has been rather good.

They also have working Asterisk sip.conf samples on the faq:
for pre-1.4 here -> http://support.viatalk.com/index.php?_a=knowledgebase&_j=questiondetails&_i=123&nav=+%26gt%3B+%3Ca+href%3D%27index.php%3F_a%3Dknowledgebase%26_j%3Dsubcat%26_i%3D42%27%3EInstallation+Guides%2FTech%3C%2Fa%3E+%26gt%3B+%3Ca+href%3D%27index.php%3F_a%3Dknowledgebase%26_j%3Dsubcat%26_i%3D60%27%3EAsterisk+Setup+And+Configuration%3C%2Fa%3E (http://support.viatalk.com/index.php?_a=knowledgebase&_j=questiondetails&_i=123&nav=+%26gt%3B+%3Ca+href%3D%27index.php%3F_a%3Dknowledgebase%26_j%3Dsubcat%26_i%3D42%27%3EInstallation+Guides%2FTech%3C%2Fa%3E+%26gt%3B+%3Ca+href%3D%27index.php%3F_a%3Dknowledgebase%26_j%3Dsubcat%26_i%3D60%27%3EAsterisk+Setup+And+Configuration%3C%2Fa%3E)
and 1.4 here -> http://support.viatalk.com/index.php?_a=knowledgebase&_j=questiondetails&_i=151&nav=+%26gt%3B+%3Ca+href%3D%27index.php%3F_a%3Dknowledgebase%26_j%3Dsubcat%26_i%3D42%27%3EInstallation+Guides%2FTech%3C%2Fa%3E+%26gt%3B+%3Ca+href%3D%27index.php%3F_a%3Dknowledgebase%26_j%3Dsubcat%26_i%3D60%27%3EAsterisk+Setup+And+Configuration%3C%2Fa%3E (http://support.viatalk.com/index.php?_a=knowledgebase&_j=questiondetails&_i=151&nav=+%26gt%3B+%3Ca+href%3D%27index.php%3F_a%3Dknowledgebase%26_j%3Dsubcat%26_i%3D42%27%3EInstallation+Guides%2FTech%3C%2Fa%3E+%26gt%3B+%3Ca+href%3D%27index.php%3F_a%3Dknowledgebase%26_j%3Dsubcat%26_i%3D60%27%3EAsterisk+Setup+And+Configuration%3C%2Fa%3E)

I haven't had the chance to work with this yet, but it looks promising. Price on service is good and I have only had a few problems over the last 18 months I've been a customer. I heavily suspect those problems were related to my ISP however.
