Title: What HD tuner card to use that is known working?

Yeah.  I finally get to build a new system and would like a recommendation for the
best (known working) tuner card that also does HD tuning (US).  Basically, I will
just be connecting the cable in (from comcast and no box) to the card
and then outputting to TV (possibly some recording to HD) but want
to be able to get all the same channels that I can currently get on my
SHARP AQUOS (which has ATSC and QAM tuner builtin).

The HDHomerun by  Silicondust is a very good way to go.  It's connected via network and fully supported by Myth.  IT is ATSC and QAM only.

Are there any USB 1080p devices that LMCE can use?

That can deliver video to the PVR, which LMCE can distribute later to the MDs?

nope, and USB 1080p is problematic over USB, don't go this route. the bandwidth concerns will overload the host controller and the amount of interrupts generated will increase your load average markedly and lower your computer performance.
